There is no substitute for a consultation and examination by Dr. Constantian here in the office. However, for patients who live more than three hours away, in order for us to help you, we need you to do the following things:
- Write Dr. Constantian a letter (no more than 1 page) stating the following:
- How many nasal surgeries you have had.
- The dates of your surgeries.
- Include copies of your operative reports (if obtainable).
- What you hope to achieve should you have another surgery:
- Is your breathing blocked?
- What do you dislike about your nasal appearance?
- Make sure that your name and address appear on your letter.
- Enclose good quality, full face photographs of:
- What your nose looks like now, including a frontal view, both side (profile) views, both oblique (¾ views) and an inferior view (with your nose facing up to show the nostrils).
- Photographs of what you looked like before you had your first surgery, if possible. Remember, Dr. Constantian can only advise you well if he has good pictures!
- Make sure that ALL photos are labeled with your name and the date that it was taken. Also, tell us if you would like your photos returned to you.
DO NOT SEND PHOTOS VIA E-MAIL.
SEND YOUR LETTER AND PHOTOGRAPHS VIA THE POSTAL SERVICE, FEDERAL EXPRESS, OR UPS. EMAILs WILL NOT BE ACCEPTED.
Dr. Constantian will review your letter and pictures ONCE EVERYTHING REQUESTED ABOVE HAS BEEN RECEIVED, and will answer you in writing, usually within two weeks.
If he believes that you qualify as a good surgical candidate, he will advise you to call the office so that we may schedule your consultation.
