In New Hampshire, near Boston, Massachusetts - Providing Brow Lift, Otoplasty, and Neck Lift

At the International Center for Rhinoplasty, we specialize in nasal surgery procedures. This page discusses some of the additional facial surgery procedures that are performed at our New Hampshire practice, north of Boston, Massachusetts. Brow lift, otoplasty, or neck lift surgery can provide significant aesthetic enhancement for many patients.

Brow Lift

The amount of hooding in the upper lid depends partially upon the position of the eyebrow. If you look in the mirror and push your eyebrow down, you will see that the upper lid looks looser; and if you raise it, loose skin in the upper lid often smoothes out, sometimes completely. The eyebrows should normally sit about ½" above the upper edge of the bone that forms the eye socket. If the brows have become droopy with time, the upper lid may appear hooded even if it is smooth; or the position of the eyebrow may make a puffy or wrinkly upper lid look worse than it really is. If the brows are very low, the excess skin over the upper lid is not really eyelid skin but eyebrow skin, and cannot be optimally improved by eyelid surgery alone. A brow lift done at the same time raises the brow, smoothes the upper lid, and makes the eye look larger. Brow lift also can reduce frown lines between the brows or smooth the transverse creases that cross the forehead. The procedure is performed through an incision in the hairline or in the scalp and can be done at the same time as eyelid surgery or facelift. Even if a brow lift is performed at the same time as another procedure, both procedures are still done as an outpatient.

Otoplasty  (Ear Pinning)

Most ears that are “big” or “prominent” have two imperfections in varying degrees, each one a failure of the last stage of embryonic development in ear growth.  The first is a flattening or unfolding of the curvature in the upper part of the ear (the superior crus of the antihelix). 

By curving the upper part of the ear back, that fold normally emerges.

Secondly, the inner cupped part of the ear (in which the ear canal is located), the concha, is not positioned close enough to the side of the head.  Occasionally the concha is also too deep and needs to be reduced. 

At our New Hampshire center near Boston, Massachusetts, otoplasty is performed with the goal of improving both of those developmental flaws. Through an incision hidden in the crease behind the ear, permanent sutures pull the conchae back toward the tissue behind the ear; and other permanent sutures create the normal antihelical curl in the upper part of the ear.  These two maneuvers, combined with a small amount of skin excision, bring the ear back to a more normal position.

"Are there any possible problems after this surgery?"

Infection and hematoma are generally uncommon following otoplasty.  Postoperative pain is usually minor; most children need no pain medication.  Occasionally, because of the elastic spring of the ear cartilage, a slight amount of the deformity may recur.  Often little bits of recurrence do not need correction because it is uncommon to see both of a person’s ears exactly at the same time, from exactly the same angle; usually one or the other is more visible.  In addition, there may be little differences between the ears preoperatively.  They may be impossible to entirely correct, so small postoperative asymmetries may also occur. 

The bandages remain in place for one week, and a protective headband is worn for three more weeks.  Otoplasty is a relatively simple operation that is rewarding for the patients that decide to have it done.  “Big ears” are unfortunately an object of teasing and ridicule, even today.  The otoplasty is a relatively straightforward correction.

"At what age should the operation be performed?"

I do not perform otoplasty until the child wants it. In other words, I will not perform an otoplasty on an unwilling child whose parents bring him or her in.  It should be the child bringing the parents in.

Some parents believe that this is unnecessarily strict, but there are two reasons that the child benefits from it.  First, if the child does not want the surgery, the whole ordeal would be a battle.  Secondly, in the unlikely event of minor complication, the parents would feel horrible guilt for having forced the child into something that he or she did not want, only to have a postoperative problem.  Otoplasty should be done when the child is ready, not when the parents are ready.  That is hard for some parents to understand, particularly if they have had prominent ears themselves and want to spare their children teasing at an early age.  In the long run, it has proven to be best for everyone.

Neck Lift

The intermediate procedure between the submental lipectomy and the full facelift is the "neck lift," which adds the incisions in front of the ear and allows the surgeon to actually remove neck skin, thereby giving the neck a tighter, smoother result in some patients. The difference between the neck lift and the full facelift is that the cheek dissection is much more limited and therefore the recovery is somewhat (although not a great deal) quicker. Still, if the cheeks are "fine," there is no reason to perform a full facelift and make things more complicated or expensive than they need to be.
